Dear Sir/Madam,
I Mr. Pankaj Agarwal, having a Saving Bank Account in Canara Bank, Rajpur Road Branch,Dehradun, A/c No. 1182101016801, I had lost the Debit Card of the same account on 15-06-11 and reported the same to the bank.
The Bank had deducted the amount of Rs. 150/- for Blocking the use of lost Debit Card from my account on 22.06.11.
After which I personally went to the Bank to get a Duplicate Debit Card.
After several requests and personal visits to the bank, on 03.08.11 the bank again deducted Rs. 165/- from my account on the account of reissue of a new Debit Card.
But since 03.08.11 till date I have not received any Debit Card from the Bank.
I have been to the Branch 6-7 times personally, leaving my job, but sometime they use to say bring your Pass Book, sometimes bring your Cheque Book or bring your Address Proof, etc.etc.
I request you to kindly take a strict action against the same.
