Respected sir,

i have a wealth + health plan [B]( policy No.15166064 ) of Rlife. policy issue date is 10.09.2009. my wife (abhilasha pandey) had suffering from stomach pain so i had admitted them date 28.12.2009 and i just inform to S.S. manager (akhilesh dwivedi) of Rlife branch Banda U.P.. when we discharge from hospital,i had submitted all paper of treatment in Rlife office banda. but they had not give me any information nearabout six month. after 6 month. when i go to office then they say me that you give again all papers of treatment then i give again all paper of treatment in Rlife office banda. in the say way i had give three (3) times all paper to office . at least office say me that you give me original papers then i give.

To date 21.05.2011 , i received a letter , i this letter he say that As per Discharge Summary the complaints for which the patient was hosptalised have been there since 20.11.2008 whereas in discharge summary a clear date of hosptalised (28.12.2009).

sir . i send two registered letter (date 08.06.2011and 06.07.2011) for above mistake and i send a Legal notice date 24/08/2011 . but not any reply tillnow.
sir now i am tired and i have mentally, economically and physically upset. please solve my problem .
