DEAR SIR,

Myself Subhash Chandra Jain , R/O 30/2/10, swarna path, Madhyam Marg, Mansarovar, Jaipur working in K.S. Motors, jaipur.I had taken one ICICI Prudential Life Insurance Policy no. 09353364 three years back. I had already deposited my two primium amount of Rs. 24000/-. This year it was due in March 2010. I had received one call from number 91132223802, who introduced himself that he is from ICICI Prudential and that your premium is due and if you will deposit it before 30th March than you will get benefit of Rs. 5.00 lacs in Medicalim. The person has called 2-3 times from the same number.

On 24.03.2010 I again received the call from same number and they have called me that about eleven o clock their agent will come and i can give the payment to him. I told them that right now I don't have cheque. He told me I can make cash payment and he will give you the receipt.

On 25th March Mr. Ramakant Verma at 2.30 P.M. visited my office, collected cash from me and gave me the receipt. I matched the receipt with my old receipts which were incidentlly with me. They we identical so i was satisfied. He told me if any problem occur you can contact me on the same number.

Generally I receive SMS on my mobile of payment received but this time after 10 days when i not received the SMS I contacted on their toll free number9829277766, I had talked to Ms. Saroj, who told me to wait for 20 days. but she also not given any satisfactory reply.

On 17th May 2010 I visited ICICI's Apex Mall, office in Jaipur and met Sh. Ojas, he also told to wait for 10 days.

During that period i have contacted several times with them but they have given not a satifactory reply.

On 26.05.2010 I have received a call from Mr. Sudhakar who told me that he required the photocopy of the receipt. Some Mr. Vijay Panwar collected the photocopy from me who’s number is 9529034446. He again called and asked for my policy number.
On 19.06.2010 I received a call from 918066848300 that your complaint is rejected as that the slip you have received doesn’t matches from our serial numbers.
Sir, I am feeling cheated and want to complain through you. Sir I question that how come that person who collected premium from me have all the details of me , he knew all my office address and my policy number and premium amount and all and above all how come he had that ICICI prudential’s deposit slip with him which matches with my earlier slip.
Please look into the matter and help in recovering the amount of Rs. 24000/-. Sir I am working in private concern and Rs. 24000/- is a big money for me. Please take up the matter with ICICI Prudential Life Insurance and recover my amount.
