Dear Sir,

Myself Gautam Shankar Seth applied for a lic policy ( Jeevan Anand) through Lic agent Ravi Shankar Choubey (Agent Code-24995412). I have Given a Cheque for Rs. 6601/- in favor of LICI to my agent as per premium Calculation for Sum assured Rs. 1 lac (Premium mode-Yearly) and filled the complete application form with my sign and all the necessary information along with my documents in the month of April 2011. after a month my agent come to and give a receipt where it is showing that the sum assured is Rs. 2 lacs, Premium Mode is Half Yearly and premium amount is approx 12000/-. I told that this wrong when I personally fillup the form then why this is happed then he told that now nothing can be done. Then I immediately submit a complain against the lic agent in the concerned lic branch on 30.05.2011 and also tell all the things to the branch manager Mr. T burnwal ( Sr Branch Manager Burra Bazar Branch Kolkata) then hi told me to talk to the concerned Development Officer for the Cancellation of the policy then I have also submit a application for Cancellation of the policy because the premium is too high and i am not able to continue the policy but till date i \have not received any revert on this..and also note that i have not received the premium recipt or lic bond yet from my agent. the agent is not picking the phone. he is totrlly out of reach and i am continuesly visited the lic branch but there is nobody is ready to resolve the issue. Please look into this and do the needful as soon as possible because i am totaly helpless.
