I am Bharat Bhooshan Dwivedi resident in Jawahar Singh Ki Payega, Nai Sadak, Lashkar, Gwalior - (M.P.). I had applied Family flotter policy from ICICI PRUDENTIAL LIFE INSURANCE CO. LTD. My Policy No : 11432015. I had suffring in Sliped Disk Problem since Dec. 2009. I had reached in network hospital (JANAK HOSPITAL, Gwalior) for treatment but still they are denied to give medical coverage to me against my policy. I had already given lots of complain to Customer care, I am also ready to submit all the relavent documents to claim mediclaim for previous & further medical treatment. Either give me my policy amount back. Need your kind co-operation to give me my justice and give medical coverage to me for my further treatment
