Dear Sir,
As per offer made by your Delhi office, I had agreed for taking the life insurance for Kotak Super Advantage in my name (i.e. Bimal Kumar Srivastava) and submitted all the documents for the same.
I received the policy document [Kotak Super Advantage (UIN-107L058V01)-Policy No. 01993639-Client Id 53473808-Bimal Kumar Srivastava] on 27th May 2010. However, to my horror, I found that your office has insured the life of my wife Mrs. Shashi Bala Srivastava, instead of insuring my life.

I immediately gave a call to the concerned official of Kotak Life Mr. Rakesh Narang who had got the forms filled, and told him the mistake done by M/S Kota life.

The very next day i.e. on 28th May 2010, I submitted the original Policy papers my Kotak Super Advantage (UIN-107L058V01)-Policy No. 01993639-Client Id 53473808-Bimal Kumar Srivastava, to your Delhi office (Through Mr. Rakesh Narang), with a request that the above policy is required to be insured in my name (Bimal Kumar Srivastava) and NOT repeat NOT in my wife’s name as she is only my nominee.

However, so far we have not received the policy back after the change of name. Thus I do not have any policy with me at present. It is therefore once again requested that the following changes may please be made in the policy, and atleast the original policy may be returned to me.
