Madam/Sir,
I Meena Singh, 240/a patliputra colony, patna-13, had opened 2 in nos. policies( 0047924720 and 0047925443) in march 2007 through Bajaj Allianz Exhibition Road, patna, with annual premium rs 25000 each. i had given cheques( SBI 252855 and 252856) to pay the third premium for 2009 but it was not paid for the above mentioned policies and without informing me office has opened 2 in nos. new policies( 0125931921 and 0125930462) with the annual premium rs 25000 each giving all fake documents/ signature. resulting my previous policies( 0047924720 and 0047925443) have lapsed and office asking to pay the 2 premiums for each.
So, it is my humble request to you kindly look into the matter and solve the problem.
