2) That my cousin late Madhup Kumar was the policy holder in Life Insurance Company Gulabbag branch Purnea (Bihar).In an unfortunate accident of flood in the yearAugust 2008 he passed away. He has left his wife and his two small children in this world.
3) After his death his wife is the only nominee who deserve the financial benefits of his policy from Life Insurance Company.About two years has been passed away ,but his wife has not been received any amount from Life Insurance Company.
4)In this contest, it has to communicate you that all necessary documents has been produced by the wife of late Madhup Kumar as and when it demanded by the company authority to concerned branch i.e GULABBAGH BRANCH, BRANCH CODE-5001
