My agent is Smt Indrani Banerjee : 88213430 .Servicing Branch No: 430
I wanted to change the address of Policy No 426296409 (Plan 179 Policy Term 20).
My agent asked for a signed application letter and photocopy of current address for submission to LIC office at Baranagar, Kolkata-7000036, to carry on the address change.
I, in hurry and mistake and out of sheer trust of long association with Smt Indrani, handed over a blank page signed by Samik Raha to the above mentioned agent on 21 Sep 2011.

She is denying to give me a signed confirmation now that she took that signed paper from me for address change in presence of Amitabha Ghosh her assistant.

I said that I would write the application but she insisted that there is a specific format for address change which she would write on that paper and submit. I also emailed her a scanned copy of passport with current address for her to take print and submit as a proof.
As confirmed over phone from her, she said that the letter has been submitted to LIC Office Baranagar on 24 Sep 2011.

Now I am fearing that whether this is true or not! Is any such application at all required to make such address change or an agent of her level can do it in a casual manner without any documentation!
I earnestly request you to please find out and confirm to me what are the original documents that have been submitted against this policy in LIC Office so that I can breathe in relief.
