Wrong debit from credit card of icici bank

Default Wrong debit from credit card of icici bank
hello
my icici bank's credit card no. is 3770 403836 68002
and icici lomard has generated one personnel accidental policy of no. 4018/IPA_71786/00/000 thru telephonic call
which was not genuine and which was canceled/closed on 20/05/'08, from icici lombard records but in the records of icici credit card they are showing outstanding amounts to be paid.
so my question is why without asking permission to me how bank can go for transactions

kindly see this matter

Dear Sir
Till date I cannot got a icici bank credit card & icici bank health Plan policy paper.icici bank credit card department wrongly debited amount on my icici salary acount
