Sub: Notice regarding Reimbursement of Mediclaim under
Policy No.140300/48/09/12/00002821
Ref.: Your letter No.140300/SDM/2010-11/
dated: 02-06-2010

Dear Sir,

Kindly refer to your above cited letter and the Mediclaim submitted by the undersigned amounting to Rs.17,935/- during the month of November,2009 for getting the operation of Cataract of my left eye at Anand Hospital & Eye Centre. I would like to bring the following few facts for your kind consideration:

1.0 That it is a matter of regret that despite of repeated requests and reminders you have not yet released the payment of the operation charges incurred by the undersigned as on date. As per the provisions of the notification issued by the Govt. of India, the earlier Mediclaim Policy being operated with an Insurance Co. can be got changed with another Insurance Compay as and when desired by the client and that can be continued with all previous benefits being extended by the previous company.

2.0 That I had obtained a Mediclaim Policy from New India Insurance Co.Ltd. having Policy No. 330100/48/06/20/70001133 and later on got changed with United Insurance Company Policy No.140300/48/07/97/00002490 during the year 2007. Copy Appended at Annexure-A.

3.0 That the policy obtained from the New India Insurance Co. has been operated since 2002-2003 and for the last 9 years no claim had been submitted to your company.

4.0 That your Agent Mr. Padam Chand Jain had been specifically asked to get the Mediclaim Policy operated with the New India Insurance Co. shifted/transferred/ changed with the United Insurance Co. and after his assurance that the previous policy being operated with the New India Insurance Co. will be continued with United Insurance Co. with all previous benefits given by the said company.

5.0 That vide your letter dated 02-06-10 you have refused to release the payment of the Mediclaim incurred by the undersigned.

6.0 That despite of my repeated requests and reminders you have not released the payment of the charges incurred by the undersigned on getting the operation of Cataract of my left Eye I have left no alternative but to take up the matter in the court of law for which you will be fully responsible for releasing the complete charges of the suit filed by the undersigned and will be liable to make the payment of compensation as well as payment for the harassment to the undersigned.

7.0 That vide this letter you are again requested to kindly consider my request to release the payment of the claim submitted by the undersigned within a period of 15 days failing which I shall be compelled to file a suit against you for non-making the payment to the undersigned and also for the injustice, harassment and fraud being done by you.


This may please be treated as a NOTICE for 15 days and you are requested to kindly release the payment of the undersigned atonce failing which you will be fully responsible for the consequences.
