Sub: Non receipt of copy of corrected Scheme Certificate issued (Scheme certificate no.MH/BAN/64684)


I received scheme certificate issued by The EPFO Mumbai I (Bandra),Bhavishyanidhi Bhawan,341,Bandra (East),Mumbai 400 051,on 24/04/08 in the name of Mohan Sankar Raman where as my correct name is Mohan Raman.

In this regard I wrote several letters to EPFO Bandra making following submission
to issue scheme certificate with correct name:
(1) That the form 10C and form 2 has been duly submitted with the correct name
namely Mohan Raman through my ex organization.

(2) Accordingly I was also receiving the EPF statement in the correct name i.e. Mohan
Raman . Similarly my pension contribution were also being remitted to regional PF office Mumbai in the correct name namely Mohan Raman.

(3)That I was an employee with Lupin Ltd.from 13/09/1986 to 22/01/05 and relieved of my services on 22/01/05 My employee code with organization was 00171972. My EPF account no. was MH/12465/X/3178 with Lupin Laboratories Ltd EPF trust, 159 CST Road ,Kalina Santacruz (east) Mumbai 400 098.


(6) That I had sent written letter earlier also on 20/04/09 along with copies of supporting / documentary evidence for all above points followed by reminders but till date I have not received any response.

After a long time i received a letter on dated 27.04.11 stating that the correction has been made and the copy of corrected scheme certificate is enclosed with the letter . However no certificate was enclosed. Till than i have written several letters and reminders to them stating the fact and requested them to send the corrected certificate but in vain. I did not receive any response and as a result till now i did not receive the corrected scheme certificate.

I have to prefer claim for pension and not getting justice till now.Pl help me in getting a copy of required corrected certificate.
