I, Deepak Kumar Singh having EPF account no DL/33814/69, would like to draw your kind attention regarding above subject matter that, I had applied for withdrawal of my EPF and EPS through filling of form no. 19 and form no. 10C, in 2009 and the form was received at EPFO office in the month of Jun’09. But-

1. When no intimation concerning settlement, rejection of such withdrawal was received from EPF office, I again applied for the same with filling of form no. 19 and form no. 10C and the same forms were received at EPF office on 21st Mar’2011, with Claim ID: DSNHP110300034144.
2. I am surprised to know through intimation (No: DS / NHP / 121 / REJECTION, Dated 26 / 09 /2011, after receiving of claim forms on second time from EPF office), form EPF office that, your claim has been settled on Date 24/ 09 / 09 for Rs 7849 and 3570, with no details mentioned except.
3. Because, neither any detail such as check No., date at which amount credited to account, account no to which amount was credited, if amount deducted from EPFO account etc. nor, as per knowledge, any such amounts has been credited in my account,
Sir, we therefore request you to please suggest into the matter in detail and help me to settled my above grievance and amount be credited in my account.
