Dear Sir,

This has reference File no.-CHDZ63155810 was applied in month of June-2010 after that police verification & all mandatory such as required had been done.

After the all legal requirement personally I have mate to passport office. They told me that your ID are matching with vide file no.-LKOA10168206, due to lack of knowledge I did not mention my previous file no. in passport form. which was applied from Lucknow in 2006. & Passport has not been issued to me,so Affidavit have been submitted in passport office Chandigarh on Nov-2010. Again I have personally mate to passport office in july 2011-.But till now I have not received passport .Status in internet is being shown as File Reference Number (CHD-Z631558-10) is either wrong or Information NOT available[/COLOR]. I have also mail in twice to chief passport officer & public relations officer Delhi but no response I've received.

So kindly I request to you plz do needful & issue my passport at the earliest.
